What should I do if my NHI Card is lost or damaged, or if I need to change my name/photo/UI No.?
You can bring the following documents to the National Health Insurance Administration Offices, post offices, household registration offices, or district offices to handle the procedures:
- A recent 2-inch half-length, front-facing, bareheaded photo without tinted glasses, with clear facial features (natural expression, not exaggerated). The photo must not be altered, and composite photos are not allowed. Free photo services are available at NHI offices’ service counters, but background retouching is not provided. Photos can be provided on a CD, but USBs are not accepted.
- Original identity documents of the cardholder (and agent, if applicable).
- A handling fee of NT$200.
